Uninstall the add-in. Use REGEDIT to make sure the add-in is completely removed from the keys HKEY_CURRENT_USER\Software\Microsoft\Office\11.0\Outlook\Addins, HKEY_CURRENT_USER\Software\Microsoft\Office\Outlook\Addins, and HKEY_LOCAL_MACHINE\SOFTWARE\Microsoft\Office\Outlook\Addins. Then reinstall the add-in from scratch.

Of course, anyone experiencing this issue and not using Office 2003 should change the "11.0" in the above to whatever version of Office they're using.
